1.) On or about November 6, 2020, approximately 4 employees tested positive for COVID-19. Proper contact tracing was not conducted to identify other company employees who were exposed to and/or interacted with the four positive employees on the days preceding the test results. Employees were informed to report to work if they were not experiencing COVID-19 symptoms, regardless of their exposure/interaction with the positive employees.Alleged Hazards: 1, Employees Exposed: 50
