1. Ceiling tiles have been hanging from the ceiling by the restroom for about two months. The ceiling tiles and support frames could fall and strike an employee. 2. The flooring at the entrance of the store is lifting up and causing a trip hazard. 3. Employees are getting bitten by mosquitoes due to a mosquitoes infestation within the entire store. 4.
