1. A room housing a COVID-19 positive patient does not have signs and/or labeling on the patient room door when transmission-based precautions (i.e., contact and airborne precautions) were in place. Employees, including but not limited to cleaning staff, entered the room without being informed of necessary precautions. This has also been occurring with other infectious disease patient rooms.
