1. An employee that was recently identified as being COVID-19 positive worked while having COVID-19 symptoms. This employee quarantined for only 7 days, and then returned to work while displaying symptoms of COVID-19 for 4 more days. The employee did not wear any face covering when they returned to the workplace. This does not follow CDC guidance and exposes other employees to COVID-19 in the workplace. 2. Employees do not wear masks in the workplace, which does not adhere to CDC guidance. 3. Adequate sanitization of the employees work area and within a 6 foot perimeter of the employees normal work area did not occur when the positive case was identified. This may expose employees to COVID-19 while in the workplace. 4. CDC guidance regarding social distancing of 6 feet or more is not maintained within the workplace. Meetings are held where employees are within 1 foot of one another. Employees regularly sit within 2 feet of one another when in their normal work positions.Alleged Hazards: 4, Employees Exposed: 25
