1. On the 11th A/B floor, COVID-19 positive patients are not being treated in negative pressure rooms. The ventilation intake in the wall is not being vented to the outside. The filters at the intakes are not being changed. These were positive pressure rooms for the immune compromised. Aerosol-generating procedures take place in these rooms 2. On the 11th A/B floor, COVID-19 positive patients have portable ventilators placed in their rooms. The ventilators are venting to the hallway. The filters in the units are not being changed. The filters are not the correct particulate size. The filters have a MERV rating of 8 instead of 16. Employees have not been trained on how to properly handle these units to ensure that negative pressure is being maintained. 3. Employees have to enter the dirty room and bypass dirty personal protective equipment to obtain clean N95 respirators.Alleged Hazards: 3, Employees Exposed: 10
