1. The employer does not offer the hepatitis B vaccination series to employees who could reasonably be anticipated to have exposure to blood or other potentially infectious materials. 2. Employees with exposure to blood or other potentially infectious materials (OPIM) are not trained annually on the bloodborne pathogens standard. 3. The containers holding cleaning chemicals used by employees are not properly labeled. 4. The fire alarm is not tested. 5. Fire extinguishers are not visually inspected monthly, and at least one fire extinguisher is expired. 6.
