1. The employer failed to notify the Occupational Safety and Health Administration (OSHA) within twenty four (24) hours of a work-related incident resulting in an in-patient hospitalization that occurred on July 27, 2020. 2. The employer requires employees to perform hazardous chemical analysis with a fume hood that does not function properly in the laboratory (Bleach Plant A-Side/SVP Operator Area); exposing employees to chemical hazards from chlorine dioxide and other chemical compounds. 3. The employer requires employees to perform hazardous chemical analysis with a fume hood that does not function properly in the laboratory (Bleach Plant B-Side/Helper Area); exposing employees to chemical hazards from chlorine dioxide and other chemical compounds. 4. The employer failed to ensure employees were practicing social distancing, maintaining six (6) feet distance, between persons within the facility or wearing cloth face coverings when other social distancing measures are difficult to maintain as in accordance to CDC (Center for Disease Control and Prevention) guidelines for the COVID-19 (coronavirus) pandemic. 5. Employees are exposed to health hazards related to the employer not informing them of when other employees have tested positive for COVID-19 within the facility as in accordance to CDC guidelines.
