1. The employer failed to notify employees within 24 hours after becoming notified of a positive COVID-19 case in the workplace. The employer waited at least three days before informing those employees who had been around the COVID-19 positive person in the establishment, and did not conduct contact tracing to determine who had been exposed. 2. After being notified of a positive COVID-19 case in the workplace, the employer did not quarantine those exposed employees. 3. The employer did not ramp up disinfecting of surfaces after having a positive COVID-19 case in the workplace.Alleged Hazards: 3, Employees Exposed: 5
