1. Covid-19 thermometer/temperature reading guns giving false readings at shift check-in, employees still allowed to work 2. Thermometer probes re-used at shift check-in 3. Face masks re-used for one work week 4. Gowns are re-used, shared, and stacked on top of each other for storage on all wings of facility 5. Brown paper bags used to help with decontamination/re-use of face masks kept in common areas, some kept in the sunlight 6. N-95 respirators are not being used on Covid-19 wing at facility 7. Employees are not fit tested for use of N-95 respirators 8. Damaged and unsafe ceiling in Covid-19 wing at facility 9. Employees do not wash hands or change PPE, breaching infection control practices, while handling clean linens 10. Change in resident health status not communicated timely to employees on all shifts, i.e. droplet contamination 11. The employer has not provided Covid-19 training to all employees 12. The employer has had employees work with healthy residents and positive Covid-19 residents on the same day 13. The employer has not provided training to employees for PPE usage or how to properly don and remove it safely
