1. An effective Infectious Disease Preparedness and Response Plan has not been developed or implemented after Covid-19 cases were confirmed. Upon knowledge of a positive test at the workplace, the employer did not identify and quarantine workers that might have been exposed. The employer provided face covers but the use was not enforced. 2. The employer has not provided adequate sanitization in the facility, common areas and work areas after confirmed COVID-19 positive cases. 3. Employees are exposed to dust due to housekeeping issues in the warehouse. 4. Employees operating forklifts has not been trained or certified.Alleged Hazards: 4, Employees Exposed: 15
