Located at Primrose School of Virginia Beach South, in rooms 1, 2, 10, 11 and infant rooms, the employer does not follow Hazard Communication Program (HAZCOM) requirements in that, 1. Containers of disinfectant/cleaners, to include spray bottles, are not labeled with chemical identify and hazards. 2. Disinfectant/cleaners (bleach-mix) are not mixed in ratios recommended by the CDC or in accordance with manufacturer's instructions. 3. Personal protective equipment (PPE) is not required or available when using bleach-mix spray solution. 4. Inadequate ventilation in areas where bleach-mix spray solution is being used.
