It is alleged that: 1. Employees and providers have been observed when they are within six feet of patients while providing services to patients or in customer facing areas without wearing face coverings, over a period of days from May 28th though the present. In addition, employees have been observed removing their face-shields and/or face coverings while still within 6 feet of other employees and/or patients. 2. Patients are not required to wear face-coverings when receiving services and are not required to observe social distancing. 3. Employees are not following social distancing of six feet. Employees are huddling together without masks. 4. Employees are being exposed to patients who are not being pre-screened before entering the facility and/or before receiving services, to determine if a patient has been experiencing any symptoms of COVID-19 or if they have tested positive for COVID-19. 5. Employees who are known to have had an exposure to COVID-19 or who tested positive for COVID-19 are still being required to work and are not being promptly sent home and told to self-isolate for at least 14 days until completely free of symptoms per the CDC and VDH guidelines 6. The employer is not conducting and does not have a schedule for properly sanitizing frequently touched surfaces and is not conducting any deep cleaning when it is reported that a patient or employee was exposed to COVID-19 or tested positive for COIVD-19. 7. The employer is not providing an adequate supply of sanitizer for employees to use to clean hands, equipment, or patient areas between patient treatments and is not allowing employees to wash their hands when they deem it to be necessary.
