1.) Employees are potentially exposed to novel coronavirus COVID-19 due lack of adequate isolation guidelines as per public health and medical requirements. Contrary to the Occupational Safety and Health Act of 1970, Section 5(a)(1). 2.) Injuries or illnesses requiring treatment beyond first aid are not being recorded as per OSHA's recordkeeping standard. Contrary to 1904.4. 3.) Employees are potentially exposed to crushing hazards due to unsafe storage of material. Materials are stored on shelves which have not been appropriately anchored. Contrary to 1910.176. 4.) Employees are not provided personal protective equipment which is required for use by the employer. Contrary to 1910.132. 5.) Employees are potentially exposed to radiation hazards due to lack of radiation surveys to ensure employees are not exposed above an average of 100 picoCuries per Liter over 40 hour work week. Employees are potentially exposed while working in enclosed spaces when repairing radiation mitigation systems. Contrary to 1910.1096. 6.) Employees are potentially exposed to crushing or engulfment hazards while working in permit-required confined spaces due to lack of a confined space program which includes elements such as but not limited to methods to identify permit-required confined spaces, and training employees on the roles and responsibilities of working in confined spaces.Alleged Hazards: 6, Employees Exposed: 10
