1. The employer does not provide face masks and does not require them to be worn to minimize the spread of COVID-19. Customers are allowed to enter the site without face masks. 2. Meetings are held in small rooms with multiple persons and no face masks are being worn. 3. Employees are allowed to come to work while having COVID-19, which exposes other employees to the virus. 4. Cranes lift heavy metal overhead and employees are not wearing hard hats. 5. Employees cutting metal with torches do not have eye protection/goggles or respiratory protection. 6.
