1. Adult Unit: A patient was admitted without having a COVID-19 test prior to arrival. When asked, the Medical Director stated that the patient had a COVID test prior to having surgery approximately one week prior to admission to Natchaug. The patient underwent surgery and had been admitted as an in-patient on a surgical floor of a hospital for approximately one week which can increase the risk of developing COVID-19 if exposed to the SARS-CoV-2 virus. The pre-surgery test is not sufficient for admission to Natchaug due to the potential exposure to the virus at the prior facility. The patient is not on Droplet Precautions or quarantine and is allowed to walk freely in the unit, at time, unmasked.Alleged Hazards: 1, Employees Exposed: 20
