1. An employee is burning incense in the ladies restroom close to chemicals. A complaint was made to the Mayor's Action hotline email address-action@shelbycountytn.gov On February 8, 2021. No attempts have been made to investigate the claims. 2. An employee does not wear a mask while working. 3. The waiting area is not sanitized after servicing clients. 4. There is a bad bug infestation problem. There are dead and alive water bugs and huge cockroaches that are just lying in the floor of the facility. They are even in nurse's evaluations rooms. Bugs and ants fall out of the AC vents. 5. Cawthon Clinic also has a mold problem.Alleged Hazards: 5, Employees Exposed: 368
