1. A COVID-19 pandemic plan has not been developed and implemented for employees to include employee wellness checks, disinfecting and sanitizing, social distancing, personal protective equipment, employee return to work and employee training. The building and restrooms are not cleaned thoroughly on a consistent basis. Several employees have tested positive for COVID-19. Employees are not informed if they have been in close contact with someone who has tested positive for COVID-19.
