An employee began exhibiting symptoms of COVID-19 on 4/22. Prior to exhibiting symptoms, the employee worked evening and late-night shifts on April 9, 12, 13, 16, a day shift on April 19 and part of a morning shift on April 22. If you visited the location during this time, please report it and any other place you may have recently visited.
