The Employer has not implemented an effective fall prevention program. Contrary to 29 CFR 1910.28. 3) Employees are potentially exposed to fall hazards associated with the use of ladders. It was reported that employees are using ladders that are not in a condition that the manufacturer would authorize. Contrary to 29 CFR 1910.23.
