1. Employer has failed or refused to provide to provide requested OSHA 300, 300A, and 301 logs/forms in a timely manner. 2. Employer failed to maintain proper negative pressure engineering controls in Covid designated unit. 3. Employer informed staff they may have been exposed, but failed follow
CDC guidance for Covid testing strategy. Employer did not require staff to be tested, recommend testing, or even offer free testing. Employer also did not require exposed staff to be quarantined. By failing to take these actions the employer created an unsafe risk of an outbreak among staff and patients at the hospital.

1. On the psychiatric unit one north it was discovered that a patient with massive symptoms of cough, pink eye and vomiting was tested positive again. The manager called every worker after they finished their shift at home and told them the patient was tested negative and
we all an relax. The chart is showing that the patient is positive. The manager did not send anybody home for quarantine for safety.

On October 21, 2020, union representative requested an up to date copy of the employer's OSHA 300 logs to see if there were any ongoing or recent COVID-19 outbreaks among nursing staff. Under OSHA the logs are due by the next business day. As of the date
of this complaint, nearly a week later, there has still been no response from the employer. As a result, employees are unable to assess whether there has been any COVID-19 infections among staff in order to effectively and timely address any resulting safety issues.

1. The third floor of Ascension Borgess Hospital, specifically 3 North Cardiology, has a disproportionate number of employees testing positive for COVID-19. The numbers of infected employees are higher than any other unit. There is concern that either poor workmanship is directing air from COVID positive patients'
rooms via the return air system, or that the air that is vented to the outdoors from the negative pressure rooms is being sucked into the intake of the heating and cooling system. Management refuses to test the air, nor will they test the employees to see if there is an asymptomatic carrier.

1. I was working on the Psychiatric Nursing Unit when a patient was showing signs of COVID. I found out that he went to medial with a positive COVID test. Less than 24 hours later, he was brought back on the unit with the hospital saying that
he had tested negative twice. When he came back on the unit he was still hacking, coughing up stuff, snot coming out of his nose, and looking horrible. I was told by the manager that I could not be tested because the doctors deemed him to not be contagious.

stop any further activity with this entity. I found a phone number for the company Subgenix Products in the bottle. 800-609-4174 and called it. The receptionist answered in a foreign language which sounded oriental and I also heard what sounded like Spanish. When I requested an English speaking person to assist, they hung up the phone. I called again with the same results.
