1. There is a sign on the front desk ordering employees to mop/wipe surfaces with pine sol and bleach which are clearly labeled stating do not mix or it will form chlorine gas. 2. Employees are not instructed/trained on how to prepare/mix the chemicals used, hazards of the chemicals used or what to do if it was to get on their skin or eyes. 3. Chemicals are mixed in a chemical sink and the room is very dark with poor lighting. 4. There are no SDS's for the cleaning chemicals used. 5. PPE is shrugged off as an optional item for all interactions with cleaning chemicals. 6. Chemicals are stored with food items and this same room is also used for lunch breaks. 7. Throughout the day, the same rags used to clean the bathroom are also used to clean the treadmills and common touch-points, staff are told to only get a new rag if the old one is used to wiped up visible pee or excrement.
