1 - Recently a vendor repaired or refinished floors that were still producing indoor solvent chemical vapors without adequate ventilation, during the workday and employees suffered adverse health symptoms. 2 - There are 4 employees, 2 case managers, a business manager, and a teacher, who regularly have lunch together in an enclosed room approximately 10 feet by 10 feet. They eat and drink within 6 feet of each other. These employees are violating the employers Covid Safety and Health plan to protect employees and others in the building from Covid-19 infection. One of these employees suffered Covid-19 illness approximately April 2, 2021, testing positive. 3 - Some of the larger occupied rooms in the facility do not have door locks, for instance the Teacher's Lounge. In the event of a Shelter in Place or Active Shooter incident, these rooms could serve as a Shelter in Place location as part of the employer's Shelter in Place or Active Shooter Emergency Action Plan if they had locks that could be locked from the inside.
