From Web Complaint # 32240772: About 13-14 people (less than 1 foot from each other) at 20x4 conference table at the elevated office trailer (VA construction site trailer - 11500 Wilshire Blvd., Los Angeles 90025) since June 9, 2020, all of last week (Monday, August 3 through Friday, August 7) and Monday (August 10), Tuesday (August 11), Wednesday (August 12) and today (Thursday - August 13) of this week from 6:00 am to 4:30 pm. Not only were these field employees not practicing the social distancing (of at least 6 feet), they were not wearing protective face masks. The employees are also not following general safety practices at the site and at the office. Just as recently as 2 weeks ago, 2 laborers contracted COVID-19 (during the swing/graveyard shift) and Frontier Kemper did not inform LA Metro (client). Also, numerous injuries to laborers since May 25, 2020 have not been reported to LA Metro (client) nor OSHA. Employees are not practicing 6 feet social distancing or wearing masks. Everyone is too close. Water is available to wash hands in the restrooms but not in the kitchen. OSHA has already provided a warning letter about portable toilets. I have filed a Fed OSHA whistleblower complaint. I take time off every two weeks. (Complainant was specific this statement be mentioned in the complaint). One employee pulled a pocket knife on me. I reported this to the HR department and the police. About 2 weeks ago, a laborer was struck in the arm by a piece of rebar and was seriously injured. I believe he was hospitalized and his arm was partially fractured. The rebar was in the concrete pad for the vertical headwall for the tunnel boring machine assembly in the vertical shaft. I assume Lockport cares is possibly food poisoning people. Lockport cares gives you bag lunches , breakfast and dinner.

For 2 nights there is always corn or brownies during Bible study at 8pm at night.

You are not required to wear mask indoors.amd of you are positive for covid 19, you are not required to wear a mask. There is a woman positive for covid 10 not wearing a mask everyday spreading her disease. Lockport Cares allows kids and married couples to stay the night also.

Lockport cares should be shut down for biohazard. Covid positive wear no masks. A mask is not required for covid positive people.

These reports of patient burn injuries described up to and including third-degree burns requiring intervention which may lead to prolonged hospital stay, scarring, and additional surgeries in both pediatric and adult patients. Severe burns could lead to potentially long-lasting impacts on patients especially under the age of 12 years.

Since that notification, we have continued to monitor device performance through post market surveillance, investigated every event reported and evaluated potential root cause of these events. Megadyne recently received a report of pediatric use that included a serious 3rd degree patient burn. We are providing this additional broad public communication to increase awareness that MEGADYNE™ MEGA SOFT™ Universal and Universal Plus products should not be used for patients under the age of 12 years.
