1. It is alleged that at least one (1) employee have test positive and all employees were not notified at all and/or within a timely manner of a potential indirect and/or direct coronavirus (COVID-19) exposure or encounter. 2. The employer is not following the current CDC Guidelines, such as but not limited to, the Interim Guidance for Businesses and Employers to Plan and Respond to Coronavirus Disease 2019 (COVID-19). 3. Proper disinfecting and cleaning is not being performed in accordance with CDC Guideline, including high touch areas, and after having a positive COVID-19 case within the facility. 4. Employees, are working in close proximity to other employees, and are not consistently practicing social distancing (6 feet distancing) in accordance with CDC Guidelines when working and moving throughout the facility.
