A COVID-19 pandemic plan has not been developed and implemented for employees to include employee wellness checks, disinfecting and sanitizing, social distancing, personal protective equipment, informing employees of possible exposure, employee return to work and employee training. The employer did not inform employees working in direct contact with COVID-19 positive employee. The employer does not require employees to wear a mask. The employer did not quarantine an employee who tested positive for COVID-19.
