1) A conductor was called in and came in contact with several other employees while known to be infected with COVID-19. The employees in contact with him were not protected. 2) The employer is not conducting contact tracing to identify and protect employees exposed to active cases of COVID-19. 3) The employer allows COVID-19 positive employees to work with other employees for 48 hours before quarantining the employee. In Fostoria, OH, an employee was found to be infected with COVID-19 on November 15th and allowed to work for two more days, coming in close contact with another employee, until November 17th. The exposed employee was not quarantined. 4) In Bellevue, OH, employees are not required to wear masks to protect against the spread of COVID-19 while indoors, including the breakroom where employees report to when arriving to and leaving the facility. 5) Employees are not screened before beginning their shift. Temperatures are not checked and symptoms are not assessed.
