1. Employees are exposed to electrical hazards in that the lid on the motor box of the lift used on the 8598 side is broken exposing the wires inside.
2. Employees are exposed to hazards associated with COVID-19 in that management does not enforce the company's face covering requirement when employees are unable to a maintain six foot distance during tasks such as, but not limited to, opening and scanning packages.
3. Employees are exposed to stuck by hazards in that Amazon pallet routinely come in unstable with heavier boxes on top and over six foot tall.
4. Employees are exposed to electrical and fire hazards in that an extension cord is plugged into a surge protector located at the supervisors desk.
5. Employees are exposed to hazards associated with COVID-19 in that cleaning of highly touched surfaces in not conducted on weekend shifts.
6. Employee are exposed to hazards associated with COVID-19 in that employees are not being provided with new face coverings when requested.
