activity on site. Observed about 15 small flies located on the ceiling and shelves around the 3 compartment and about 10 small flies located in the rear office by the utility sink. Instructed to clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 5-205. 15 observed a leak at the faucet neck of the rear food prep area's hand washing sink. Instructed to repair.

- comments: 6-501. 12 observed floors thru out the rear under and around the 3 compartment sink and floors under the front customer counter with accumulated dirt debris. Instructed to clean.

I did not order the item I received. There is no name of the company to return it to and the item is pretty much useless as it has a British plug on it. A month ago my Visa was compromised with 15 charges attempted on it. I don’t know if these 2 things are connected but I’m thinking they are
