Located at Accordius Health dba The Citadel Virginia Beach, where approximately 25 residents and 5 employees have tested positive for COVID-19, employees are exposed to the following alleged hazards: 1. The isolation unit has been taken away and COVID-19 positive residents are dispersed throughout the facility 2.

During the evenings and weekend, employees are not following COVID-19 isolation protocols and are permitting COVID-19 positive residents on isolation to leave their rooms. Management is aware of this and has not taken disciplinary action. 3. Employees are purposefully completing pre-shift COVID-19 questionnaires incorrectly to be permitted to work 4. Management is not verifying that pre-shift questionnaires and temperature checks are being completed as required 5. Frequently touched surfaces are not being cleaned and disinfected at least once during or at the end of the shift
