properly cleaned and disinfected due to a lack of oversight by management to ensure accountability of the custodial staff who performs the cleaning. 3. Management no longer ensures contact tracing is being done to notify employees of possible/potential COVID19 exposures so employees can possibly quarantine and seek testing. The issue is accountability, the mail and the mission of moving mail has become primary over the loss of life or potential loss of life.

Also, a door in the back of FSS is chained shut.

utilize the area at the same time and throughout the shifts. Cleaning supplies are not made available for production employees to use for sanitizing the breakroom before touching surfaces that have been contaminated by other employees.

2) Employees are allowed to comingle in very close proximity to each other in the breakroom.

3) Throughout the facility, equipment, machines, and other surfaces that are used by multiple employees throughout the shift are not sanitized between contact by different employees.

4) Management is not ensuring that housekeeping personnel are effectively cleaning surfaces on a regular basis throughout the facility, especially following the report of a confirmed case of COVID-19 in the work area.

5) Management is not contact tracing or notifying employees in a timely manner of potential exposures that may have occurred at the facility based on suspected cases or confirmed reports of COVID-19 positive employees.
