People not using gloves while preparing food. No carbon monoxide monitor. Unclean floors. Face masks not being worn all the time. Equipment broken and not being fixed when needed.

Employer is not informing staff of positive COVID-19 cases in the facility. Employer is allowing COVID-19 positive tested employees come into work early from quarantine if symptom free.

They are bringing in recovering COVID-19 patients and do not have proper ventilation systems -Negative Room Pressure in place. They "rigging" the existing system to try to prevent exposing the many elderly residents in the facility. Told that we have to bring COVID-19 patients in by law... of State Health Dept. and CDC.
