Aisles 1-23. Racks that store pallets are damaged to the point where they are unsafe to use, exposing employees to struck-by hazards. Product hangs over the racks and vertical supports near the floor are bent and twisted. Applicable OSHA Standards: 29 CFR 1910.176 and Section 5(a)(1) of the OSH Act 3. Powered industrial trucks, including standups and sit-downs, frequently stop suddenly during operation, power steering is lost and operators have to slam on the brakes. A wrench icon appears on the vehicle’s computer panel signaling the need for vehicle maintenance. When this occurs, operators are instructed by management to over-ride the safety feature by turning the vehicle off and on again to reset it and continue operating, rather than taking the vehicle out of service for repairs. Also, to avoid taking powered industrial vehicles out of service, vehicle problems are not always noted when completing daily/shift inspection information on the vehicle's computer panel. Employees are exposed to struck-by and crush hazards. Applicable OSHA Standard: 29 CFR 1910.178. 4. The employer does not ensure that employees authorized to operate powered industrial trucks and pallet jacks are adequately trained in the safe operation of the vehicles. Operators involved in powered industrial truck accidents are not removed from driving and provided with refresher training and performance evaluations (driving tests) before being allowed to operate vehicles again. Employees are exposed to struck-by and crush hazards. Applicable OSHA Standard: 29 CFR 1910.178 5. Dock locks (North Side) are faulty at approximately 15 dock doors and do not secure the trailers to the dock. The locking arm on the dock lock system does not move upward to secure the rear of the trailer to the bar when the button is pushed. When this occurs, an alarm goes off and a manager is called to enter a safety over-ride code into the system and loading/unloading with powered industrial trucks is permitted. Employees are exposed to struck-by and crush hazards. Applicable OSHA Standards: 29 CFR 1910.178 and Section 5(a)(1) of the OSH Act
