1. Respiratory protection is not provided to employees cutting and grinding on concrete components (e.g. bridge pilings), thus exposing employees to respiratory hazards. Location: Yard Area 2. Hand protection is not provided to employees handing liquid concrete, thus exposing employees to skin irritation hazards. Location: Yard Area

3. Hearing protection is not provided to employees working around high-noise producing machinery (e.g. concrete pourers), thus exposing employees to noise hazards. Location: Yard Area 4. There are pieces of strap metal with sharp edges scattered across the ground, thus exposing employees to laceration injuries. Location: Yard Area 5. Appropriate eye protection is not provided to employees conducting oxygen/propylene torch cutting operations on steel braided cables, thus exposing employees to optical radiation hazards. Location: Yard Area and Welding Department 6. There are approximately eight (8) oxygen and propylene gas cylinders that have damaged and corroded gas cylinder pressure regulators, thus exposing employees to a fire hazard. Location: Yard Area and Welding Department 7. Local exhaust ventilation is not provided to capture welding fumes, thus exposing employees to respiratory hazards. Location: Welding Department 8. There is an accumulation of concrete dust on surfaces (i.e. walls, desks, and floor) within the office, thus exposing employees to respiratory hazards. Location: Quality Control Building 9. The male restroom is not maintained in a sanitary condition in that there is feces, dust, and dirt on the walls of the restroom, thus exposing employees to biological hazards. Location: Adjacent to the Quality Control Building 10. Soap or a similar cleaning agent is not maintained in the male restroom, thus exposing employees to biological hazards. Location: Adjacent to the Quality Control Building 11. The employer has not provided silica training to employees that handle concrete, thus exposing employees to respiratory hazards. Location: Yard Area 12. Deep puddles of rainwater are located throughout the ground area, thus exposing employees to a slip, trip, and fall hazards. Location: Yard Area 13. The employer has not implemented the Centers for Disease Control and Prevention (CDC) guidelines as they relate to the Coronavirus Disease (COVID-19) into the workplace to include providing face coverings and practicing social distancing by ensuring employees maintain six (6) feet from each other. Location: Throughout the Workplace
