Onn 3 outlet power outlet tap (Model Number: ONA-12AV049) malfunctioned causing the unit to arc within the outlet receptacle.
Caused external burns to the wall outlet, melted the wall outlet, damaged the wall outlet plate, as well as caused severe burns to the prongs of the unit... See More in which an indent and corrosion occurred on the device itself from the malfunction. The device malfunction caused the outlet to burn out. Onn device was being used to connect two lamps to the wall outlet. This was the only device plugged into the wall receptacle. Avoided injury, however would not have if the outlet wasn't the first in a circuit which made it so that a device on the other side of the room wouldn't work that was plugged into a different outlet.
It was during my investigation trying to find out if there was perhaps a GFI reset somewhere that I didn't remember that I discovered it turned out to be that this device had began to burn and arc causing the wall outlet to actually melt and burn as well. The unit when still plugged in was crackling and buzzing inside of it separate from the wall outlet and the surge protection light was still lit as if it was functioning properly but wasn't. The outside of the device had no preexisting damage or misuse. But following there is also melting on the back of the product where some of the raised lettering had been singed off partially during the malfunction. Had we been out or asleep when this occurred, a catastrophic loss of property and life would have possibly been imminent.

My wife and I went to Costco on Saturday 10/29/2022 at approximately 10:00 am. We left approximately 10:30 am and after driving about five minutes the low tire air pressure light came on and the left front was showing as fifteen pounds. We immediately drove to the... See More Hyundai dealership because we have a tire warranty with them. When we got there we were told that someone had stabbed the sidewall of the tire with a knife. We tried to think of why someone would do that to our car but couldn't think of a reason. We had not jumped in front of anyone for the parking spot and were parked within the lines. As we were talking to the cashier before leaving, she suggested that someone might have thought there was a female alone in the car. My wife does have a front license plate with her name on it so that did sound like a possibility. If that was the case, then they probably changed their mind when I walked out of the store with her. With the kidnapping and such going on I thought that you might want to take a look at the Costco video from that time period to see if it matched up with any cases you are working on. We called the Costco manager later and reported the incident. He said he would look at the video and let us know if he saw anything. We have not heard from him. See Less

5 days ago, I began usage of CVS Salicylic Acid Wart Removal Strips that contain 40%
salicylic acid. I have used them for about 5 days. Over the last five days around day two
I began having bouts of nausea but never attributed it to this. The... See More following day began
stomach cramps and pain. Today my left side of my back is burning deep inside
around the kidney area. I have excessive thirst not being fixed by drinking water. My
vision is blurry and I also have an awful taste in my mouth. The taste in my mouth is
exactly what the scent of these strips is. I looked up and then contacted a poison
control center and it is suggested this may be a case of Salicylic Acid poisoning from
this product. The occurrences over this short span ae far too coincidental for this to not
be the case. Upon further research it is nearly identical to a case in 2008 in which a
pediatric patient exposed to a similar product experienced similar symptomatology that
was determined to be a poisoning by this type of product. These products are not safe.
The product has been used on the left pointer finger below the middle knuckle. There is
some area of skin that has sloughed off and was bright pink earlier and is now white.
The finger itself feels off like neurologically off and partially numb currently. It has now
been 24 hours since I began this report and the symptoms are beginning to subside
slightly. It is a slow subside though. The only thing different today is my kidney does
not hurt constantly now it is off and on but much less intense and easing as time goes
on more so. The nausea has subsided. The taste however still remains in my mouth but
is slightly less than before but not enough yet. The other remaining symptoms include
stomach cramping, some nausea off and on but no longer constant, bloating, I
experienced measured tachycardia as well as a burning sensation along the left hand
side of the mid back right below the rib cage ending. This pain has subsided but is still
occasionally present for a split second then gone. | Symptoms: Cramps, Blurry vision See Less
