1. No face masks / face coverings are provided to protect against COVID-19. 2. No cleaning and disinfecting supplies provided to protect against COVID-19. 3. Bars are positioned across doors that people should be able to walk through. The bars are never taken off. This condition is in building 3144. 4. Employees are exposed to a trip hazard created by a hole in wall that has a board at bottom. 5. Employees are exposed to mold on the wall under the air conditioner located inside the Warehouse of building 3144. 6. Air vents in the Men's and Women's restrooms are filled with dirt/dust and are in danger of falling from the ceiling above the toilets. 7. Employees have not been provided OSHA training for over a year. 8. The employer does not have an effective extermination program in place. Employees are exposed to large rats in the building. 9.
