1) No measures were taken to protect employees from exposure to COVID-19 after a resident tested positive for COVID-19. 2) Employees who are showing signs of COVID-19 are allowed to continue to work. 3) Resident rooms are unclean and residents have to sit in feces on their toilet. 4) Medication is left out and residents who aren't prescribed the medication have access to it. 5) 3rd shift is allowed to sleep on the clock.

I don't know how this got my card number and ordered this bracelet of some kind. Brand new card never used it but to pay a bill with my phone. And I received a call from my bank asking if I used my card I was driving and said I haven't ordered anything, don't know how or why
