a failure to properly screen out staff who had close contact with positive cases or who reported experiencing COVID-19 symptoms themselves. Youth who tested positive were moved to a confinement unit (A2) but were not moved to the medical quarantine unit (D2) designed specifically for this situation and located directly off of the medical unit. Security staff working the quarantine unit are assigned to the unit for eight hours and are often assigned to a different unit the next day (no consistency leading to COVID-19 spreading to other units). Medical staff work on both the quarantine unit and throughout the facility on the same shift as well Hazard Location: Throughout the facility at IYC-Chicago but specifically on the youth living units. On Tuesday, March 9, 2021, one female staff who worked the unit where the outbreak happened, tested positive and other female staff stated she was at home the Monday night prior, but was told she was ok to report to work. One of the managers sent out an email saying due to them not working in the past few days, no other staff needed to be screened which is not true. One staff was working the Saturday prior which is when she probably e3xposed after telling the supervisor that the youth she had was sick and the nurse telling the youth he could refuse to be tested. The Director of the Dept. of Juvenile Justice sent out an email saying that if you have any symptoms to stay home. The Superintendent and Assistant Superintendent are still denying staff the proper quarantine protocol. Also, there are 2 youth in particular on A-unit who refuse out right to wear their mask and the administration refuse to make them. The youth gets family visits and are part of individual outside groups who could easily bring COVID into the building. Chief of Security put out an email stating if any staff writes/reports close contact with a youth they would be issued an oral reprimand. The Assistant Superintendent actually told the staff who tested positive that if she says she was in close contact with a youth, she would be written up.

When I tried to contact customer service, several times, they do not even reply! They will send you a confirmation email saying a ticket has been opened but then you hear NOTHING. I'm furious because this is being presented on many levels as a supposed way to earn extra cash, even on articles about retired ways to earn money.
THIS IS A SCAM! They will get you to spend money to play the games thinking that you will earn more than you spend but you will NOT and NO ONE will answer questions at all.

reports of lacerations, two of which required medical attention, including stitches. These injuries occurred during assembly or cleaning/handling of the product.

The choppers were sold at Walmart stores nationwide and online from August 2022 through October 2023, priced between $10 and $15. The recalled product is the Mainstays Electric Mini Chopper with model number MS14100094536S1. These choppers are rechargeable and cordless, featuring a green lid, white body, and clear plastic bowl. They come with a single blade attachment, which includes three curved blades on a white plastic spindle. The model number can be found on the label attached to the bottom of the clear plastic bowl.

Consumers should immediately stop using the recalled choppers, and contact Walmart to receive a full refund. Consumers can bring the chopper to their nearest Walmart store for a refund.
